Dox Health Cancellation and Refund Policy

This policy ("Policy"), a part of Dox Health Terms of Service, outlines the terms and conditions for cancellation and refunds related to services provided by Dox Health, Inc., its affiliates, and subsidiaries (referred to as "Dox Health," "us," "we," "our," or "Company"). Our services encompass the use of the Company's websites, e.g., www.qper.health, www.qperhealth.com, www.qperhx.com, or www.doxhx.com etc. (the "Site"), the Company's mobile application, known as the QPER Health App (the "App"), as well as any hardware and software devices offered by the Company (collectively referred to as "Devices"). The Services include all other content, products, or offerings made available by the Company through the Site, App, or Devices.
Cancelation of subscription plan and refund
Purchases from Independent Retailers
If you purchased compatible devices or products from an independent retailer or another online retailer or any third-party, please follow their respective refund guidelines. Dox Health does not endorse or recommend any independent retailers, online retailers, or third-party sellers for blood glucose monitors and accessories, blood pressure monitors, or digital weighing scales. Our Users are required to buy their own FitBit or Apple Fitness watch or tracker which are not available for purchase on QPER Health Sites. The return and refunds of the FitBit or Apple Fitness watch or tracker bought from FitBit or Apple or any third-party are subject to their terms and policies.
Renewal Cancellation
After the initial twenty-one (21) days from the start of your Subscription Plan, you may cancel the automatic renewal of your subscription plan at any time before the renewal date. This can be done through the QPER Health website. Your subscription will continue until the end of the current subscription period and will terminate after your current active subscription period concludes. No partial or pro-rated refunds for subscriptions will be issued after the initial twenty-one (21) days period from the commencement date of your subscription.
Defective devices
If your QPER Health Devices are experiencing malfunctions due to defects in materials or workmanship within the first twelve (12) months from the date of delivery, Dox Health, Inc. will conduct an inspection and, if necessary, may replace your defective product. This replacement is subject to the terms and conditions outlined in your warranty by the manufacturer of the Devices. For detailed information, please consult the warranty provided in your User Guide.

If you have purchased compatible devices from a retailer other than Company Sites, please be aware that your ability to return these products, receive a refund, request repair services, request exchange, or pursue other remedies is determined by your agreement with the third-party retailer, and it is not governed by Dox Health, Inc. To the fullest extent permitted by applicable law, Dox Health, Inc. holds NO liability or responsibilities concerning your interactions with any third party.
Please note that the warranty provided in your User Guide by the manufacturer is the exclusive warranty offered by Dox Health, Inc. This warranty supersedes all other warranties, whether expressed or implied, including any implied warranty of merchantability or fitness for a particular purpose. The warranty provided in the Product User Guide applies exclusively to the original purchasers of Dox Health Products through the Company Sites. It does not extend to the performance of a Dox Health Product that has incurred damage due to accidents, alterations, misuse, tampering, or abuse in any form.
Blood Glucose Accessories Delivery Plan ChangeBlood Glucose Monitor Accessories Delivery Plan orders placed online through the QPER Health Sites can be changed online, upgraded or downgraded wherever applicable, before the order has been processed by logging into your account on the Site and navigating to change accessory delivery plan. Once the order has been processed and shipped however, it is not eligible for change for the current accessory delivery period. The change will take effect for the next automatically renewing accessory delivery period. Note that as long as you have an active subscription, the minimum number accessories delivery included with the subscription will renew and fulfill automatically at intervals described in the Site.
